
Passport Information
The District Clerk’s Office accepts applications for passports Monday through Friday from 8:00 a.m. to 3:30 p.m. Appointments are not necessary
APPLICATION PROCESS
To apply for a U.S Passport, you must:
Complete form DS-11 in black ink. You may get this form from our office or online.
1. Present Proof of U.S. citizenship with one of the following:
Previous U.S. Passport
Certified copy of birth certificate issued by city, county or State, (no hospital birth certificates with footprints)
Naturalization Certificate
2. Present Proof of Identity
Current valid Driver’s license/Identification Card
Previous U.S. Passport
Naturalization CertificateMilitary ID
For Minors:
Children age 16 and under, BOTH parents must be present. If one parent cannot be present they must fill out a DS-3053 and it must be NOTORIZED. Your child MUST appear in person.
If your child age 16-17 does not have identification of his/her own, you need to accompany your child, present your identification, and co-sign the application.
Pay the applicable fees
Passport Application fees: see attachment
Pay the Passport processing fee to the “U.S. Department of State” see fee page
Pay the execution fee to the “Milam County District Clerk”(separate checks)$25.00
